Consent Form

 

You must be at least 18 years old to participate in this survey.

This survey involves a web- based experiment designated to understand the relationship between ego identity motives and alcohol use among college students. The study is being conducted by principal investigator Dr.Julie Stokes, Department of Psychology, and Co-investigator Layin Freeman, second year graduate student in the clinical psychology program at California State University, Fullerton.

 

Participation in the study typically takes 50 minutes and is strictly voluntary. All responses will be kept anonymous, and the data collected will be used for research purposes; only group outcomes will be reported. All responses will be kept on a secure disk drive. All data records will be kept confidential to the extent provided by law.  You may refuse to answer any question or terminate your participation at any time without penalty or loss of any benefits that can be reasonable expected. Also, if you are uncomfortable answering any questions in this survey, you may choose not to respond to those questions.
