Informed Consent Statement for Research Participants


(Please note: You must be at least 18 years of age in order to consent to participate in research projects.)

     This survey, entitled "Brief Survey of Counseling", is being conducted by Dr. Lisa Mori, Professor of Psychology at California State University, Fullerton (CSUF; Dr. Mori is a tenured faculty member in the Psychology Department. She is a clinical psychologist who has also been a licensed psychologist in California since 1989), in conjunction with the CSUF Counseling & Psychological Services (CAPS) Center. This is a 10-15 minute survey focusing on your perceptions of counseling or psychotherapy. Participation involves reading and answering questions related to counseling, as well as providing background information about yourself, such as your gender, age, education level, household income, ethnicity, as well as your personal preference for language and about your interactions with your own and other ethnic groups. You will also be asked about your own experiences, if any, with psychological conditions and counseling. Results of this study may be helpful to CSUF adminstrators and CAPS staff in resource allocation and planning, other college counseling center personnel, as well as to mental health professionals in improving outreach and psychological services to the general public.
     You will take the survey online at your own convenience and direction. It is estimated that the survey will take about ten to fifteen minutes to complete.
     Your participation in this survey is entirely voluntary. You may refuse to answer any or all questions or terminate your participation at any time without penalty. If you have questions regarding your rights as a human participant in research, please contact the CSUF Institutional Regulatory Board Compliance Coordinator at (657) 278-7640; the office is located in MH-112. If you should become distressed during or after your participation in the survey, you may contact Dr. Mori, the principal investigator and a licensed clinical psychologist, at lmori@fullerton.edu or by telephone at (657) 278-3761. If you are a student at California State University, Fullerton, you may also contact the Counseling and Psychological Services (CAPS) Center for assistance at (657) 278-3040. CAPS is located in the Student Health Center - East.
     The information you provide will be confidential and used for research purposes only. Only those individuals directly involved in the study will have access to the data. No one will be able to link the information you provide to your identity in any subsequent public presentations or publications, as no names or IP addresses will ever be used. You will be asked to assign yourself a 4 digit code number to insure this confidentiality. Any hard copies of the data will be shredded under the supervision of Dr. Mori within one year from the date of data collection. All copies of the data will be kept in Dr. Mori's locked campus lab and office. All data/records will be kept confidential to the extent provided by law.
