Informed Consent Statement for Research Participants


(I am at least 18 years of age and thus able to consent to participate in this research project.)

     This survey involves assessing personal characteristics, attitudes, feelings, experiences, and behaviors. My participation in this study is entirely voluntary. I may refuse to answer any question or terminate my participation at any time without penalty. If I have questions regarding my rights as a human participant in research, I may contact the Institutional Regulatory Board Compliance Coordinator, Office of Grants and Contracts, California State University, Fullerton (CSUF), located in MH-112 or by phone at (714) 278-7640. If I should become distressed during or after my participation in the survey, I may contact Dr. Mori, the principal investigator and a licensed clinical psychologist, at lmori@fullerton.edu or by telephone at (714) 278-3761. If I am a student at CSUF, I may also contact the Counseling and Psychological Services Center for assistance at (714) 278-3040; CAPS is located in the Student Health Center-East Building.
     The information I provide will be anonymous and used for research purposes only. No one will be able to link the information I provide to my identity. I will assign a 4-digit code number (the last 4 digits of my campus wide identification number) to myself to insure this confidentiality. Any hard copies of the data will be shredded under the supervision of Dr. Mori within one year from the date of data collection. All copies of the data will be kept in Dr. Mori's locked campus lab and office. All data/records will be kept confidential to the extent provided by law.
     In this study, I understand that I will be asked to provide some demographic information about myself (i.e., age, ethnicity, education, etc.). Next, I will be asked about my feelings, behaviors and experiences. Specifically, this driving survey includes questions about my ethnic and occupational background, my driving history, such as traffic citations and driving under the influence of alcohol or other substances, my vehicle and typical driving patterns, such as the average number of miles driven per week and under what type of traffic conditions, my driving attitudes and behaviors in the past week, how I feel about encountering particular circumstances on the road, and my general level of stress in the past month. 
     My participation in the survey is appreciated by the researchers.
