I have been asked to participate in a research study examining how individuals view certain aspects of their competitive experience in sport. Results from this study may enhance the quality of athletic participation in the future. This research study is being conducted by Dr. Lisa Mori, Associate Professor of Psychology at California State University, Fullerton, and her research associates.
     My participation in this study is entirely voluntary. I may refuse to answer any question or terminate my participation at any time without penalty. Non-participation or withdrawal will not affect my receiving an introductory psychology or other psychology course credit slip (if applicable). Participation in this study is likely to require 15-30 minutes. If I have questions regarding my rights as a human participant in research, I may contact the CSUF Institutional Regulatory Board Compliance Coordinator at (714) 278-7640. I may or may not find the sensitive nature of some of the questions upsetting. If I should become distressed during or after my participation in the survey, and I am a student at Cal State Fullerton, I may contact the Counseling and Psychological Services Center, located on campus in the Student Health Center - East building, by telephone at (714) 278-3040. If I should become distressed during or after my participation in the survey and I am or am not a CSUF student, I may contact Dr. Lisa Mori, the Principal Investigator and a licensed (clinical) psychologist, at lmori@fullerton.edu or by telephone at (714) 278-3761.
     In this study, I understand that I will be asked questions about my background (age, gender, education, annual income, etc.) and questions regarding my views of and experience as a participant in athletics. The information I provide will be confidential and used for research purposes only. No one will be able to link the information I provide to my identity. My name will not appear anywhere in any subsequent reports, presentations, or publications that may result from this study. I will use either a self-assigned code number or (if a Cal State Fullerton student) the last 4 digits of my Campus Wide Identification number (CWID) on my completed survey to insure this confidentiality while also allowing me to receive course credit (if applicable) for my participation. Any hard copies of the data will be shredded under the supervision of Dr. Mori within one year from the date of data collection. All copies of the data will be kept in Dr. Mori's locked campus lab and office. All data/records will be kept confidential to the extent provided by law.
     Once I have agreed to participate in the survey, I will receive introductory psychology or other psychology course credit for my participation if I am a psychology student at CSUF. My participation in the survey is appreciated by the researchers.
     I have carefully read or have had this study and the terms used in this Consent Form and their significance explained to me. I am of age (at least 18 years old) and fully competent to indicate agreement to this Consent Form. If online, I understand I should print a copy of this informed consent form for my files.
