Instructions for Survey of Alcohol and Driving


Please answer all of the questions below as honestly as possible. Your answers will be kept anonymous. For the purpose of this questionnaire, the term "slightly intoxicated" refers to the condition of the average-sized person who has had four drinks within one hour. Driving while "slightly intoxicated" refers to driving within one hour of ingesting four drinks in one hour. Driving while "intoxicated" refers to driving within an hour of having more than four drinks.
