Informed Consent Statement for Research Participants

 

      This survey involves providing information about your opinions, attitudes, and learning experiences outside the classroom while studying in the methods/pedagogy courses in the MS TESOL/TESOL Certificate Programs.  My participation in this study is entirely voluntary.  I may refuse to answer any questions or terminate my participation at any time without penalty.  If I have questions regarding my rights as a human participant in research, I may contact Heidi Hodges, Regulatory Compliance Coordinator, Office of Grants and Contracts, CSUF, at 714-278-2327.  If I should become distressed during or after my participation in the survey, I may contact Dr. Eyring, the principal investigator, at jeyring@fullerton.edu or by telephone 714-278-2577.  If I am a student at California State University, Fullerton, I may also contact the Counseling and Psychological Services Center for assistance at 714-278-3040.

 

      The information I provide will be anonymous and used for research purposes only. Any hard copies of the data will be shredded under the supervision of Dr. Eyring, Dr. Carr, or Dr. Gallego within two years from the date of data collection.  All data/records will be kept confidential to the extent provided by law.

 

      In this study, I understand that I will be asked to provide some information about participation in community-based learning or experiential learning, to assess various aspects of this experience, and to provide demographic information about myself (i.e., age, gender, racial/ethnic identification, etc.). 

 

      My participation in the survey is very much appreciated by the researchers and will be valuable for assessing certain aspects of the TESOL Program.  

 

      I am a current TESOL student or an alumnus of the TESOL Program.
